A CDDO is a Community Developmental Disabilities Organization. As the CDDO for Sumner County, Futures Unlimited, Inc. serves as the primary agency coordinating area services for individuals with developmental disabilities. Working together with the other 27 CDDO's serving Kansas, Futures Unlimited, Inc. helps to assure a seamless support network is available to individuals and families wherever they may call home.

As Sumner County's CDDO, Futures Unlimited:

• Provides a single point of entry for people seeking services in Sumner County, or access to state-provided institutional services. Call 620-326-8906 or 888-326-8906.
• Determines eligibility for services following state-mandated criteria.
• Maintains a resource list of all area providers, state agencies, county organizations, and emergency numbers.
• Offers consumer directed case management services.
• Provides information and referral services as well as assistance in finding funding to support desired levels of support. In Kansas, eligibility for services does not guarantee services.
• Works with affiliated Community Service Providers to ensure customer-driven service options.
• Facilitates the CDDO area Council of Community Providers.
• Provides quality assurance and oversight of all affiliated Community Service Providers.
